Direct to patient ordering options

To better serve your practice, Myriad is offering two different processes to ship our test kits directly to your patients’ home and support the patient with any sample collection questions that may arise.

Virtual Ordering

Log in to our online ordering portal to submit the test request form electronically to indicate you want the kit sent directly to the patient.

Paper Test Requisition Form (TRF)

Email/fax a test request form to Myriad using a cover sheet that indicates you want the test kit delivered directly to patient.

Online Ordering Portals

  • Provider Portal: Foresight® Carrier Screen and Prequel® Prenatal Screen (NIPS)
    Log in to our online provider portal to submit the test request form electronically and to indicate you want the kit sent directly to your patient.

    NIPS and carrier screening are critical to comprehensive pregnancy risk assessment that allows you to manage each patient appropriately. Please ensure you have accurate billing and shipping information for each person receiving a kit. Also, make sure your patients set up a patient portal account here, so they can track sample progress and view results, if desired.

    1. Log in to your Myriad Prenatal Portal account.
      • If you are already a username, login to your account here.
      • To register for our portal, provide your sales representative with your email address and Myriad will send you an email with a link to select a password and access your account.
    2. Proceed to ordering and when asked “Do you have a collection kit?” select “No”. You will be required to provide the patient’s name and address, city, zip code and state.
      • If ordering Foresight, you will also be asked “Are you ordering for a couple?” at which time you can complete and submit an order form for your patient’s partner.
    3. The Myriad team will then send a test kit directly to the patient’s home. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope. Patients will be notified via email to expect a shipment.
    4. For Prequel, the patient will contact Client Services to discuss blood collection options and the phlebotomist will send the specimen to Myriad for processing. For Foresight, the patient (and her partner) will collect saliva samples and will send the specimen(s) to Myriad for processing.
    5. Once Myriad has received the patient sample(s), turnaround time, billing, and report delivery will proceed as normal.
    6. If you signed up for our results delivery service, we will notify your patient when results are ready. Patients can access results via the patient portal and speak with a member of our clinical Patient Education team about their results at no additional cost.
  • MyriadPro: MyRisk® Hereditary Cancer Test
    Log in to our online portal, MyriadPro, to submit the test request form electronically and to indicate you want the kit sent directly to the patient.
    1. Log in to our secure online portal, MyriadPro.
      • If you are already a MyriadPro user, log in to your account here.
      • To register for MyriadPro, click here, or for instructions on how to register and use MyriadPro – click here.
    2. Fill out an electronic test request form (eTRF), and be sure to indicate that you want the test kit delivered to the patient for sample collection by checking the box. Once that has been done, the following fields will be required to proceed:
      • Patient name and address, city, zip code, state
      • Your e-signature for authorization (check the signature box)
    3. Click the ‘submit’ button to submit the test request form. The Myriad team will then send this completed TRF along with a test kit directly to the patient’s home. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
    4. The patient will then need to complete the specimen collection and mail it back to Myriad for their sample to be processed. Once Myriad has received the patient sample, turnaround time, billing, and report delivery will proceed as normal.
    MyriadPro Registration Instructions

Paper Test Requisition Form (TRF)

  • TRF: Foresight® Carrier Screen and Prequel® Prenatal Screen (NIPS)
    1. Fill out a test request form (TRF) as you normally would – download Foresight only TRF here. For Foresight & Prequel TRF, download here.
    2. Use this cover sheet to indicate that you want the test kit sent directly to the patient’s home. Follow this guide so we can process the request as efficiently as possible.
    3. Email or fax the TRF and cover sheet to Myriad.
    4. Myriad will review and then mail your completed TRF along with a test kit directly to the patient. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
      • For our Prequel Prenatal Screen (NIPS), the patient will receive an email with instructions to call Client Services to if they have questions about sample collections.
      • For our Foresight Carrier Screen, you have the option of sending two saliva kits for both members of the couple or a single saliva kit.
    5. The patient (and her partner) will complete the specimen collection(s) and mail it back to Myriad for the sample(s) to be processed.
    6. Once Myriad has received the patient sample(s), turnaround time, billing, and report delivery will proceed as normal. The average turnaround time for sample processing for the Prequel Prenatal Screen is 1 week, and for the Foresight Carrier Screen is 2 weeks.
    7. If you signed up for our results delivery service, your patient will be notified her results are ready via email and text. She can access her results via the patient portal (before or after a tele-consult with a patient educator, depending on the result type).
  • TRF: MyRisk® Hereditary Cancer Test
    1. Fill out a test request form (TRF) as you normally would – download TRF here.
    2. Use this cover sheet to indicate that you want the test kit sent directly to the patient’s home.
    3. Email or fax the TRF and cover sheet to Myriad.
    4. Myriad will review and then mail your completed TRF along with a test kit directly to the patient. The kit will include specimen collection instructions, a pre-paid return shipping label, and a return envelope.
    5. The patient will then need to complete the specimen collection and mail it back to Myriad for their sample to be processed.
    6. Once Myriad has received the patient sample, turnaround time, billing, and report delivery will proceed as normal.
